When someone in your family is leaving the hospital or staying in a skilled nursing facility, RBYS is the team that takes them home and stays involved. We work hand in hand with the hospital, the SNF, and the insurance plan that's already coordinating their care.
Three services that work together so the person you love isn't left on their own after the hospital sends them home.
01 / Transportation
A licensed EMT crew picks up your family member from the bedside, drives them home in a wheelchair or stretcher van, and walks them inside. The same crew handles dialysis, oncology, and follow-up rides afterward.
02 / In-home visit
An EMT visits the home and a clinician joins by video to review medications, check on how they're doing, and answer the family's questions. For higher-acuity cases, a clinician comes in person.
03 / Ongoing care
Daily check-ins, remote monitoring, and the same team your family member already knows. Not a new vendor every week.
At this stage, most of our work comes through partnerships with hospitals, SNFs, and insurance plans. The fastest path is usually to ask their care team. We can also walk you through it directly.
01 / At the hospital
If your family member is in the hospital, ask their case manager, social worker, or discharge planner to refer transportation and follow-up care to Right By Your Side. We work with most hospital systems in Greater Orlando.

If your family member is in a skilled nursing facility, ask their care manager or discharge planner to coordinate with RBYS for the transition home and the in-home follow-up.

Some Medicare Advantage and Managed Medicaid plans already work with RBYS. Call the number on the back of their insurance card and ask about Right By Your Side as a benefit.

We work with most hospital systems across Greater Orlando. Ask the case manager or discharge planner to refer to Right By Your Side. If they have not worked with us yet, our team can walk theirs through how it works.
The path is the same. Ask the SNF care team to coordinate the transition home through RBYS. We work directly with SNF discharge planners to schedule the transport and the follow-up visit.
In most cases yes, through Medicare, Medicare Advantage, or Managed Medicaid plans that have RBYS as a covered provider. Specific coverage depends on the plan and the service. The case manager at the hospital or SNF can confirm.
